- Child's birth certificate (original + photocopy)
- Aadhaar card copies — child and both parents / guardians
- 4–6 recent passport-size photographs (colour, white background)
- Medical fitness certificate from a registered doctor
- Completed PGS admission application form

- Transfer Certificate (TC) from previous school — original
- Previous academic year report card / progress report
- Migration Certificate — if from another state
